Two Reviews, Or It Is Not Done

Never trust the thing that built it to tell you it is good. This sounds like paranoia until you have watched an AI confidently hand you broken work with a clean bill of health. The builder is the last thing that should be allowed to grade its own output.

The standard we hold is two reviews on everything: one from something other than the builder, and one measured check of the real result by a human. The failure mode is subtle, and this lesson names it precisely, along with the exact gate we use so review never quietly becomes optional under volume.
